1. Fail to use proper risk mgmt including:
a. risk % per trade (generally 2%)
b. proper stop losses (generally based on technical analysis or percentage)
c. overleveraging trades
2. Failure to manage and monitor active trades

In total transparency, this has more stop loss hits for me since I generally use a tighter stop than most people.
But it does get all the large moves.
